"Mr. Robert G. and Camp Smeltzer, both reidents of Stark County, Ohio, being duly sworn on our oaths, respectfully say, that we are father and brother of Radama J. Smeltzer and that we are present as members of the same household and family at the date of the birth of our son and brother, the said Radama Smeltzer up to and including June 1, 1895 the date of birth of Radama J. Smeltzer.  That the said Radama J. Smeltzer was born in the city of Leechburg, County of Armstong, State of Pennsylvania and that his parents were Robert G. and Susan B.
